sorry, i haven't read the whole digest, but i wish to suggest:
1) don't control the originating email address, but originating SMTP
address (IP e.g.), this will restrict a list user to use a constant
smtp, and if wishes to change - to change her/his list credentials.

2) OF COURSE don't allow unsubscribed to post.

BTW these spam messages of the subject for:
"RE: CAPITOLS suisos aosidfaosidu"
are running all around the world.
