Well, this is not an open-source/free option, but theKompany.com is selling an
app called tkcPhone which is a SIP based voip app for Qtopia/Opie. I have not
used it so I can't comment on it's pros/cons. It was written for the Sharp
Zaurus but should run with Opie on an iPAQ.
