It seems that lcd wasn't initialized.

Well, you should modify your startup.txt.
If you use the one from http://www.gwynux.ca/h2200/ it is customized for
ipaq h2200. You should use this one
http://www.xmission.org/~sa/h4355_linux/startup.txt or change mtype to
459 in the your startup.txt.

mtype configure your kernel initrd etc to match your hardware!!
After loading zImage the kernel is configured to work with your
hardware, and initrd load the right modules by reading machine name from
/proc/cpuinfo.

If mtype is wrong, initrd try to initialize the lcd with the wrong
modules.

Best Regards

PS for Michaelo: perhaps may be useful to rename into your site and
mirrors startup.txt to startup.txt_h2200 and create startup.txt_${model}
where model are models supported by your initrd
