PATCH: fix usb + add asic3 initialization.

Hi,
I fixed the h4000 usb so you dont have to unplug and replug to get it to
work, and you dont have to have usb plugged in at boot for it to work
(you can plug in usb after boot now :-).

Would someone with cvs access check this in for me?
