I just finished documenting the procedure for myself. Take a look. I'd

The only part of your setup that I would question is the 100 MB FAT
partition. I used 10 MB. FAT usually means FAT16, which in the good
old days was limited to 32 MB. That may or may not have anything to do
with your problems, it's just a guess.
