I had exactly the same problem 1 month ago.

I suspected from a possible misconfiguration of pcmcia modules. No luck.

I read a message from someone saying it was due to a low battery on the
pccard sleeve. I charged it completely, but the problem persisted.

Then I began test the hw. Fortunately a colleague of mine had an ipaq. I
checked with his card: same thing. Then I checked with his pccard sleeve
and it worked always! With his and with my orinoco pccard.

At that point it was clear to me that the problem was the sleeve. I
changed it and the new one worked always, until today, like a charm.

My advice: test the sleeve: go to the shop and ask for a new one. You
can test it even there (plug in to AC in the shop).
