Connector: http://www.gomadic.com/deaxcoplasxs.html
Circuit board: http://www.pad2pad.com

The advantage of Pad2Pad is two-fold. First, whoever makes a successful
board design can post the .pcb file and anyone can download it and order
it direct. Second, "We assemble components that appear in the Part tool
list of the Pad2Pad software as well as we assemble boards with any
parts available from popular vendors, such as DigiKey
<http://www.digikey.com>."

Presumably, that includes standard connectors.

I hope this helps.
